Prone Surgery Setup Checklist: Table Mat, Gel Pads & Arm Support

Prone surgery, where the patient lies face-down, is one of the least forgiving positions when it comes to setup mistakes. Get the padding wrong and the consequences show up as pressure marks, restricted breathing, or nerve damage that has nothing to do with the actual procedure. Here's a practical, step-by-step way to think through the setup before the patient is turned.

Step One: Start With a Supportive OT Table Mat

Every setup should begin with the base layer. An ot table mat placed across the full table surface gives every other piece of equipment a more forgiving foundation to work from. Skipping this step and relying only on targeted pads leaves the areas between them resting directly on a hard surface for the length of the procedure.

This base layer becomes even more important in prone positioning specifically, since the patient can't be easily checked or adjusted mid-procedure the way they might in other positions.

Step Two: Layer Position-Specific Gel Pads

With the base mat in place, gel pads get added at the specific pressure points the prone position creates. Unlike a supine setup, prone positioning shifts the major pressure points to the front of the body rather than the back, which changes where padding actually needs to go.

Getting this step right requires understanding that prone positioning isn't simply a supine setup flipped over, the pressure distribution is genuinely different and needs its own approach.

Areas That Typically Need Attention

●        Forehead and cheeks, supported separately by a dedicated head rest

●        Chest, to allow the lungs room to expand during breathing

●        Hips and pelvis, which bear a significant share of body weight face-down

Step Three: Add Prone Position Gel Pads for Chest and Hip Support

prone position gel pads are shaped specifically to leave the abdomen free while supporting the chest and hips on either side. This gap matters more than it might seem, restricting abdominal movement in this position can directly interfere with a patient's breathing throughout the procedure.

Step Four: Don't Forget Arm Positioning

Arms are easy to overlook once the torso and head are properly supported, but they need just as much attention. An Arm Rest Gel Pad keeps the arms in a stable, neutral position, reducing the stretch and pressure that can otherwise lead to nerve injury during a long prone procedure.

Common Setup Mistakes to Avoid

●        Skipping the base mat and relying only on targeted pads

●        Positioning the head rest without checking eye clearance

●        Leaving arms unsupported or in an extended, awkward angle

●        Not rechecking padding after the patient is turned and settled

Final Checklist Before Starting

Before the procedure begins, it's worth a final pass: base mat in place, chest and hip padding positioned correctly, head rest checked for eye clearance, and arms properly supported. A few minutes spent confirming this setup is far less costly than addressing a pressure injury afterward.

Making this check a standard part of every prone setup, rather than something done only when time allows, is what actually keeps outcomes consistent across different surgical teams and shifts.
