An operating table on its own is just a flat, hard surface. Everything that makes it safe for a patient to lie on for several hours comes from what's added to it, the padding, the positioners, and how carefully both are matched to the procedure being performed. Two of the most frequently used setups, lying flat and lying on the side, each need their own approach to get right.

Why the Base Layer Matters Before Anything Else

Before any position-specific equipment goes on, ot table positioning gel pads typically form the base layer across the table surface. This cushioning reduces overall contact pressure regardless of which position follows, giving every subsequent piece of equipment a more forgiving surface to work with.

Setting Up for the Supine Position

Lying flat on the back is the most common starting point in surgery. supine position gel pads are placed under the heels, sacrum, and back of the head, the three areas that carry the most sustained pressure in this position. Because this setup is used so frequently, it's tempting to treat it as low-effort, but pressure injuries in this position are just as real as in any other, they simply build up more gradually.

This gradual build-up is exactly why supine positioning sometimes gets overlooked during equipment checks. A rushed setup for what looks like a routine case is often where preventable pressure injuries actually start.

●        The sacrum, especially in longer procedures

●        The back of the head, where scalp pressure builds unnoticed

Switching to a Lateral Setup

Turning a patient onto their side changes the entire pressure map. Instead of weight spreading across a wide back surface, it concentrates onto a narrow strip along one side of the body, the shoulder, hip, and knee in particular. lateral position gel pads are placed specifically at these points to prevent the kind of localised pressure that a flat pad alone can't address.

Why a Lateral Positioner Is Non-Negotiable

Padding alone isn't enough to hold a patient safely on their side. A Lateral positioner physically supports the body, keeping the spine aligned and preventing the patient from shifting during the procedure. Without it, even well-placed gel pads can't compensate for a body that isn't properly stabilised in the first place.

Building One Setup That Works for Both

Facilities that handle both supine and lateral procedures regularly benefit from standardising their base layer first, then layering position-specific pads and positioners on top as each case requires. This keeps setup time predictable without compromising on the support each position actually needs.

It also makes staff training simpler, since the same base process applies across most cases, with only the position-specific layer changing based on what the procedure calls for.

Final Thoughts

Whether a patient is lying flat or on their side, the right combination of base padding, position-specific pads, and proper stabilising equipment is what actually keeps them safe through a long procedure. Skipping any one piece leaves a gap the others can't fully cover.
